The Family Supports Waiver (FSW) is part of Indiana’s Medicaid Home- and Community-Based Services program. It provides person-centered support to children and adults with intellectual or developmental disabilities, helping them live independently in their homes and communities.
Eligibility
To qualify, individuals must:
- Have a disability diagnosed before age 22 that is expected to last indefinitely.
- Show substantial limitations in at least 3 of 6 life areas (e.g., self-care, learning, mobility).
- Meet Medicaid eligibility requirements.
Services Offered
FSW provides up to $26,482 per year for services such as:
- Respite care, therapy (speech, occupational, physical)
- Behavioral support, day programs, transportation
- Assistive technology, case management, and more
Services are delivered by approved Medicaid providers and tailored to each person’s needs.
Waitlist & Priority Access
Applicants are placed on a statewide waitlist. However, priority is given to:
- Adults ages 18–24 transitioning out of school
- Dependents of active-duty military or veterans
